What’s Really Happening in Your Foot

If you’ve ever taken your first steps in the morning and felt a sharp, stabbing pain in your heel, you may already be familiar with one of the most common conditions we treat in our clinic: plantar fasciitis. It affects an estimated 1 in 10 people at some point in their lives, and it’s one of the leading causes of heel pain we see walk through our doors.

Let’s break down exactly what’s happening in your foot, why it happens, and — most importantly — what you can do about it.

What Is the Plantar Fascia?

The plantar fascia is a thick, fibrous band of connective tissue that runs along the bottom of your foot, connecting your heel bone (the calcaneus) to your toes. Think of it as a natural “bowstring” that supports your foot’s arch and absorbs shock every time your foot hits the ground.

This tissue is built to handle a lot of repetitive stress — but like any structure under constant load, it has a breaking point.

What Actually Goes Wrong

Plantar fasciitis isn’t a single dramatic injury like a tear or a break. Instead, it develops gradually through a process of repetitive microtrauma:

  1. Repeated overload. Every step places tension on the plantar fascia, especially at its attachment point on the heel. Small amounts of stress are normal and even beneficial, but excessive or repeated loading beyond the tissue’s capacity causes tiny micro-tears to form faster than the body can repair them.
  2. Degeneration, not just inflammation. Despite the “-itis” in the name (which implies inflammation), research now shows that chronic cases involve more degenerative changes in the tissue — similar to what happens in tendinopathies. The fascia becomes thickened, disorganized, and less resilient over time.
  3. Pain and stiffness. As the tissue becomes irritated and structurally compromised, nerve endings in the area become more sensitive, leading to the classic sharp, stabbing heel pain — especially with the first steps after rest.

How Physical Therapy Helps

The good news: plantar fasciitis responds very well to conservative treatment, and most people improve significantly without surgery or injections. As physical therapists, our approach typically targets the underlying causes, not just the symptoms:

  • Manual therapy to reduce tissue restriction and improve mobility in the foot and ankle
  • Targeted stretching of the calf muscles, Achilles tendon, and plantar fascia itself
  • Strengthening exercises for the intrinsic foot muscles and posterior chain to improve load tolerance
  • Gait and movement analysis to identify mechanical contributors and correct compensatory patterns
  • Load management guidance to help you modify activity appropriately while healing (rather than stopping completely)
  • Footwear and orthotic recommendations when appropriate
  • Taping or bracing techniques to offload the fascia during the acute phase
